Please complete, sign, and return the following forms. They may be returned to the band hall or sent to [email protected] .
Please note, the Pre-Participation Physical Examination form MUST be on file PRIOR TO the first rehearsal. Students who do not have one on file will not be allowed to participate in rehearsals until a completed PPE form has been returned to Mrs. Knight or Katie Bohl. Students who have already completed a PPE form for athletics do not have to complete a second form. |
